Neck Pain

One-on-one care for neck pain in a private South Natick studio: a full session with a Doctor of Physical Therapy every visit, for people who want to turn their head, sit through a workday, and sleep through the night without their neck deciding for them.

For the neck that has outlasted the stretches and the new pillow

Most neck pain we see has been around a while: the desk job, the long commute, the old whiplash, the shoulder that never quite got sorted. By the time people come in they have tried heat, a foam roller, a different pillow, and a handful of stretches from the internet, and they are still waking up stiff.

Neck care here is led by clinicians with advanced residency training at the Institute of Orthopedic Manual Therapy, with an emphasis on hands-on spine care, and board certification as Orthopedic Clinical Specialists. Every session is one-on-one for the full visit. No aides, no handoffs, no exercise sheet doing the treating.

In Massachusetts you have direct access: no physician referral is needed to start. If your evaluation shows something outside the scope of physical therapy, we will tell you and point you to the right practitioner.

Mobilize what's tight. Strengthen what's weak.

That is the whole approach, applied precisely. Your evaluation identifies which joints and tissues in the neck and upper back are restricted and which muscles are underperforming, and treatment works both sides of that equation every session.

Hands-on treatment for what's tight

Restoring motion in the neck and upper back:

  • Joint mobilization & manipulation of the neck and thoracic spine
  • Instrument-assisted soft tissue mobilization (Graston)
  • Deep tissue massage for the upper traps, levator, and suboccipitals
  • Dry needling
  • Assisted stretching & mobility work

Progressive strengthening for what's weak

Rebuilding the support system your neck relies on:

  • Deep neck flexor and extensor endurance
  • Shoulder blade and upper back strength
  • Posture that holds up through a workday, not just in the clinic
  • Graded return to lifting, sport, and overhead activity
  • An individualized home program that evolves with you

Neck problems we treat

  • Chronic neck stiffness
  • Desk and screen-related neck pain
  • Pinched nerve / pain into the arm
  • Herniated or bulging cervical discs
  • Whiplash and post-accident neck pain
  • Neck-related headaches
  • Muscle strain & spasm
  • Degenerative changes / arthritis
  • Neck pain with lifting or sport
  • Recurring flare-ups

Not sure what yours is? That is what the evaluation is for.

"Shawn is a miracle worker! I'd been to three different PTs before Shawn, and only he was able to figure out my issue and then recommend exercises that got me running again. I'm ever grateful."

Where it leads

The goal is not just a calm neck. It is a neck and upper back you trust again. When pain resolves, your plan progresses from rehab into performance training with the same clinician in the same facility, so the strength that protects your spine keeps building instead of stopping at discharge. That transition is the Progression Method.

Neck pain FAQs

Do I need a referral to see a physical therapist for neck pain in Massachusetts?
No. Massachusetts has direct access, so you can start physical therapy without a physician referral. Book a free 15-minute fit call and we'll tell you whether we're the right place to start.
How soon should I see a physical therapist for neck pain?
Sooner is better. A stiff, painful neck changes how you turn your head, sit, and sleep, and the shoulders and upper back start compensating quickly. If pain is limiting how you work at a desk, drive, train, or get a full night's sleep, that's reason enough to be evaluated.
Can physical therapy help neck pain that runs into my arm?
Usually, yes. Pain, numbness, or tingling that travels into the shoulder blade or arm often comes from an irritated nerve in the neck, and it typically responds to hands-on treatment that takes pressure off the nerve combined with strengthening for the deep neck and shoulder blade muscles. If we see signs that need a physician's eyes, we'll tell you.
Do I need an MRI before starting physical therapy?
Usually not. Disc and joint changes in the neck show up on scans of plenty of people with no pain at all. A thorough orthopedic evaluation of how your neck, upper back, and shoulders move tells us more in most cases. If we see signs that imaging or a physician consult is warranted, we'll tell you.
Do you take insurance?
We're an out-of-network, fee-for-service practice. That's what lets every visit be a full session, one-on-one with a Doctor of Physical Therapy, with no aides, no double-booking, and no visit limits set by a payer. Many PPO plans reimburse a portion of out-of-network care through the Super Bill we provide, and HSA/FSA funds typically apply. We'll walk you through what care costs on your free fit call.
What happens in a session?
You work start to finish with one senior Doctor of Physical Therapy. Sessions combine hands-on treatment, such as joint mobilization or manipulation, Graston, deep tissue work, or dry needling, with corrective and strengthening exercise, plus a home program you actually understand.
Can I keep lifting or training while my neck hurts?
Often yes, with modifications. Complete rest rarely fixes a neck. Part of the evaluation is working out which movements you can keep, which need a temporary change, and how to set up your desk, your pillow, and your training so the neck stops getting re-irritated.
I get headaches with my neck pain. Is that something you treat?
Often, yes. Headaches that start at the base of the skull and are worse with neck stiffness or long hours at a screen frequently come from the upper neck joints and muscles, and they tend to improve as the neck does. If your headaches have features that need a physician's evaluation, we'll say so.
