SMS Consent Documentation
How PT Progression collects consent to send text messages, published for verification. Program number: (508) 960-7935.
This page documents the exact consent experience for the PT Progression text messaging program. PT Progression, 10 Eliot Street, South Natick, MA, sends appointment-related messages (confirmations, reminders, callback coordination, rescheduling) from (508) 960-7935 only to people who expressly opt in through one of the two paths below. Consent language version: 2026-08-21.b1.
Path 1: website checkbox
The contact form includes an optional SMS consent checkbox that is unchecked by default. The form submits successfully whether or not the box is checked. The exact text shown beside the checkbox:
I agree to receive appointment-related text messages from PT Progression at the mobile number provided, including confirmations, reminders, callback coordination, and rescheduling. Message frequency varies, typically a few messages per appointment or request. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of receiving care or any service. SMS Terms | SMS Privacy
On submission we record: the phone number, the checkbox state (checked or not), the date and time, the source (web), and the version of the consent text shown. A blank or unchecked box is recorded as no consent, and no texts are sent. Providing a phone number for a callback is not consent.
Path 2: verbal consent
During an inbound phone call to (508) 960-7935, PT Progression's automated receptionist reads the following consent script verbatim before any text message is sent. The same script is used by staff for in-person requests at 10 Eliot Street, South Natick:
PT Progression would like to text you at [your mobile number] about the appointment or callback you requested, including confirmations, reminders, and rescheduling. Message frequency varies, typically a few messages per appointment or request. Message and data rates may apply. You can reply STOP to opt out or HELP for help. Consent is not a condition of receiving care or any service. May we send these text messages?
The caller must answer yes. The workflow is deterministic: the receptionist reads the complete script, asks the yes or no question, and records the result. Only a clear affirmative answer counts as consent. Silence, an unclear answer, an interruption, "that's my number," or merely requesting an appointment or callback is recorded as no consent and no texts are sent. We record: the mobile number, the affirmative or declined result, the date and time, the source (phone call or in person), and the script version.
Evidence
