The following article discusses Dr. Andrew Huberman's recent podcast episode, "Protocols to Strengthen & Pain Proof Your Back".
Overall, Andrew Huberman’s podcast provided a lot of good, easily digestible information for the lay-person in terms of general anatomy and principles for maintaining a pain-free back.
I was introduced to this podcast from numerous Physical Therapist colleagues, many of whom were critical of the podcast, calling it “dangerous” and reiterating ideologies from 20+ years ago. Huberman’s main source was Dr. Stuart McGill, who has a 30+ year history of researching spine biomechanics and is an accomplished author, however, mostly an academic with very little time actually working with patients.
Management of lower back pain has changed dramatically over the past few decades. My first exposure to the world of Physical Therapy was an internship in 2008, and since then I have seen tremendous differences in the way we treat and manage individuals with lower back pain. At this point, I feel that I have “seen it all” from pain education, passive stretching and traction, table “ab” exercises, Pilates, to the more recent approaches of heavy weightlifting and deadlifting to improve lower back pain, just to scratch the surface.
In my experience, properly managing lower back pain necessitates a multi-disciplinary approach. Most importantly, we must meet our patients where they are at, and understand each person has had their own experience with back pain that dramatically effects their beliefs, mood, relationships, goals, etc. I see myself as a "guide" to facilitate and encourage them to get back to normal and enriching lives through movement.
Utilizing this framework allows each session to cater to them, whether we do manual stretching and mobilizations, progressing with core stabilization, or re-educating faulty movement patterns. This is a dynamic process that needs to be adapted and changed on a day-to-day basis.
In conclusion, I definitely think that this podcast deserves a listen. It provides useful information for understanding lower back pain and strategies for managing it. However, as with most things in life, we need to critically think and adjust plans, especially for something as complex and pervasive as lower back pain.
